To get your rebate, just purchase one of these products between 1/23 and 3/6 and then submit the rebate form you can fill out and Print HERE.  You will need to mail the form and attach your dated register receipt and UPC number.  You must make a minimum purchase of $6.47 and the maximum amount of the rebate is $13.99 not including tax.
